Program/Intervention Description

What: An evidence informed practice to help children connect and re-connect to build networks of support. An exhaustive search to identify connections is done. The goal is to heal relational trauma and increase the ability to connect. How: There are four stages to service provision: Phase (1) Trusting phase lasts up to nine months with the goal of building trust. Exhaustive searching begins during this phase. Phase (2) Healing Phase is from six to eighteen months with the focus on healing trauma and connecting to lost love ones. Phase (3) Connecting Phase is from twelve to eighteen months focusing on connecting and healing relational trauma. Phase (4) Supporting Phase is from twelve to twenty-four months. The Youth Connection Scale is used to evaluate progress.
